C.H.A.T . Registration Information

If you are interested in enrolling in CHAT, here is what you need to do:

CHAT Enrollment Checklist:

Download and print CHAT registration forms.

Read through all information included in the handbook, code of conduct, and registration forms, including class descriptions and schedules.

Complete all registration requirements and mail to:

Your completed registration must include:

Completed Family Registration Form (new families only)

Non-refundable $25 registration check payable to CHAT (new families only)

Signed Medical/Legal Waiver Form (new families only)

Signed Student driver form (if applicable)

Volunteer Form (optional)

Completed registration form for each day

If postmarked in December: Full tuition payment of $170 + supply fees for each class registered for, with checks made out to individual tutors

If postmarked in January by January 3: Full tuition payment of $180 + supply fees for each class registered for, with checks made out to individual tutors

If postmarked after January 3: Full tuition payment of $180 + supply fees + $10 per class late fee for each class registered for, with tuition and supply checks made out to individual tutors and late fee check made out to CHAT (max late fee of $30 per family)

Separate supply fee check only if the supply fee is over $30. These checks may be cashed before the tuition checks. If supply fee check is not separate, the entire tuition check may be cashed with the other supply fee checks.

All completed registrations and tuition payments must be postmarked by January 3, 2012. Registrations and payments postmarked after that date will incur late fees (see CHAT Handbook under the "Financial Responsiblities" section).
